For veterans & their families

DVA exercise physiology, in your own home.

Gold and White Card holders can access exercise physiology with a D904 referral from their GP. I bill DVA directly, I come to your home, and I bring everything the program needs.

Yes — and typically at no cost to you.

Eligible Gold and White Card holders can access exercise physiology with a referral.

What to say at the appointment

Ask your GP for“a D904 referral to an exercise physiologist”It’s a standard DVA form your GP will have done many times. You can name Prevail on it, or leave it open and bring it to me.

Gold Card holders are covered for all clinically necessary care. White Card holders are covered for their accepted conditions. I bill DVA directly at the DVA fee, so there’s normally nothing for you to pay and no paperwork for you to handle.

A D904 covers a set number of sessions within a referral period, and your GP can renew it when it runs out. I’ll tell you when you’re getting close, rather than letting a referral lapse in the middle of a program.

I’ll confirm your specific eligibility with you before we start — no surprises.

What we work on

The things that decide whether you stay in your own home

Most veterans I see aren’t chasing a fitness goal. They want to stay steady on their feet, keep doing their own shopping, and not have the next fall be the one that changes everything.

Steadiness and falls

Balance, reaction and lower-limb strength — assessed in your own hallway, on your own back step, because that’s the only honest place to assess a falls risk.

Getting strength back

After a hospital stay, an illness or a slow few years. Real progressive load, brought to your lounge room, because a program too easy to change anything changes nothing.

Persistent pain

Backs, knees, shoulders, old injuries that never quite settled. Graded, evidence-based, and built around what you still want to be able to do.

Heart, lungs and long-term conditions

Exercise prescribed as treatment rather than advice — with objective measures, so progress is a number and not an impression.

Sleep, mood and the rest of it

Movement is one of the better levers on mood, sleep and stress. It gets worked on deliberately here, not left as a hoped-for side effect.

Keeping up with what you enjoy

The walk to the club, the garden, the stairs at the RSL, keeping up with grandchildren. If the goal is outside the house, we work on it outside the house.

Not sure whether your card covers what you need? Send an enquiry with your card type and I’ll confirm it before you commit to anything.

What actually happens

Once the funding is sorted

I come to you, with the equipment. You need about two square metres of floor and nothing else.

Week 1 — we talk before we move

History, goals, what’s changed and what you’re finding hard. Then gentle baseline measures — standing up from a chair, walking distance, balance, grip. Nothing you can fail.

Week 2 — your program, in your space

Built around your kitchen bench, your hallway, your back step. Written up in plain language with photos so you’re never guessing between sessions.

Week 4 — measured, in writing

The week-one baselines repeated under the same conditions, so the change is a number rather than an impression.

The longer version — how sessions work, why I come to you, and what’s in the car.

DVA questions

Does DVA cover exercise physiology?

Yes. Eligible Gold and White Card holders can access exercise physiology with a D904 referral from their GP, and I bill DVA directly — so there's normally nothing for you to pay.

What do I ask my GP for?

Ask for a D904 referral to an exercise physiologist. It's a standard DVA form and your GP will have done plenty of them. You can name Prevail on it, or leave it open and bring it to me.

What's the difference between a Gold Card and a White Card here?

Gold Card holders are covered for all clinically necessary care, whatever the condition. White Card holders are covered for their accepted conditions — so the referral needs to relate to one of those. I'll confirm your specific eligibility with you before we start.

How many sessions does a referral cover?

A D904 covers a set number of sessions within a referral period. When it runs out, your GP can renew it — that's routine, not a fight. I'll tell you when you're getting close rather than letting it lapse mid-program.

Will it cost me anything?

Typically nothing. I bill DVA directly at the DVA fee, so there's no gap and no paperwork for you. If anything about your situation falls outside that, you'll hear it from me before the first session.

Do you come to my home?

Yes — that's the default. In-home and community sessions across Brisbane, within about 12 km of Paddington, plus telehealth anywhere in Australia. I bring all the equipment, so you don't need a gym, a machine or a spare room.

I'm not sure my card covers this. Can you check?

Send an enquiry with your card type and I'll confirm eligibility before you commit to anything. It costs you nothing to ask, and I'd rather check than have you find out afterwards.

I'm on a DVA card and also over 65 with an aged care package. Which do I use?

Usually DVA, because there's typically no contribution. But it depends on what you're working on and what each covers — tell me both and I'll map it out with you rather than guessing.

Enquire about a place

Including if you haven’t got the referral yet — tell me your card type and I’ll confirm what’s covered before you see your GP.

Please don't attach or paste plans, assessments or medical history here. This is just enough for me to check capacity and make contact — once we're connected I'll set up a secure way to share anything sensitive.
